In this randomized, open-label non-placebo-controlled trial, 35 participants with systolic heart failure took 3 g of l-citrulline malate or no additional intervention beyond hospital care daily for 4 months. Resting left ventricular ejection fraction at rest and under stress increased in the citrulline group and decreased in the control group, the difference being statistically significant. Right ventricular ejection fraction also increased more than control both at rest and under stress, but the differences weren't statistically significant. The differences in blood pressure weren't statistically significant but there was an advantage of citrulline.
