Nicotine is one of the many, albeit most famous, alkaloid in cigarettes that naturally occurs in tobacco. Nicotine itself exists in many other plants (those of the Solanaceae family such as eggplant or peppers) but at miniscule doses. When nicotine is isolated from tobacco containing products or cigarettes, it has a significantly different profile of effects in the body and for all intents and purposes should be seen as a different intervention.

Nicotine has a few mechanisms at its roots. The first is that nicotine mimics the neurotransmitter acetylcholine and can directly activate acetylcholine receptors (which can then induce increases of catecholamines such as adrenaline a dopamine; this mechanism underlies both potential addiction and fat burning). Nicotine may also act as an anti-estrogenic compound, inhibiting aromatase and one of the two estrogen receptors directly; this may underlie some of the side-effects associated with chronic usage of nicotine, particularly in women. Finally, nicotine is pro-oxidative in nature but at a level which may be hormetic in nature, that works with the acetylcholine mechanism just mentioned to exert anti-inflammatory effects.

Nicotine appears to have a role as a fat burner due to its mechanisms, which for the most part increase adrenaline and then work through beta-adrenergic receptors (the molecular target of Ephedrine); this increase in adrenaline mediates the increase in metabolic rate which is significant but short-lived with moderate usage. The increase in lipolysis (how available fatty acids are to be burnt) appears to be mediated by other, possibly pro-oxidative, mechanisms and not by adrenaline.

This increase in catecholamines also underlies many benefits of nicotine on Cognition (attention and focus mainly) while the acetylcholine mimicking may promote a Nootropic effect inherently.

In regards to addiction, the risk of nicotine and addiction is a measurement between how much nicotine is taken (with more nicotine being associated with greater risk) and the speed of nicotine reaching the brain (with faster spikes in neural concentration being associated with both greater perceived benefits and greater risk of addiction). Addiction is not inherent to nicotine, as is evidenced by nicotine therapy being used to curb cigarette addictions. Gums and patches have less potential risk for addiction than do cigarettes (with inhalers in the middle) due to speed nicotine reaches the brain.

Acutely, potential side-effects of nicotine are similar to acute side-effects of other stimulants such as Ephedrine, Yohimbine, or Caffeine due to increasing catecholamines. Over the long term, the side-effect profile of nicotine may rival ephedrine as those two retain a degree of catecholamine secretion over time (the other two losing efficacy in 2 weeks or less).

Nicotine can be administered via several methods (excluding cigarettes, which are not recommended due to risks greatly exceeding benefits):

  • An inhaler, which lets you rapidly feel the effects of nicotine (and inherently carries more risk than other forms due to the speed at which it works)
  • Topical patches, which have about a 1 hour delay between application and absorption and maintain constant serum levels of nicotine but with less cognitive spike (least risk potential, least nootropic potential)
  • Chewing gums, which are sort of an intermediate between the two

There is currently insufficient evidence to suggest an 'optimal dose' of nicotine for non-smoking individuals.

For non-smoking individuals, it would be prudent to follow stimulant usage guidelines and start with a low dose and work up. This includes buying 2mg gums or cutting a 24mg nicotine patch into quarters to start, then work up to what is seen as the minimum effective dose. There is currently no established 'threshold' for when risk becomes too great as it varies between individuals.

If using nicotine as Nicotine Replacment Therapy (to curb smoking cravings), then following the instructions on the product is sufficient. These instructions may be excessive for a non-smoking individual.


It would be good to attempt to differentiate cigarettes (which is a cocktail of chemicals paired with a social experience) and nicotine itself, which is just one chemical in the cigarettes. Similar to how supplements can interact with each other, a compound can also interact with an experience and either enhance or alter the effects (which happens with instantaneous reward associated with the smoking of a cigarette)

I would hazard a guess nicotine would not be as hated if an alternate reality existed where cigarettes did not exist, as it does have potential to be useful. The addiction issue is less of a concern outside of cigarettes, but it still exists (although a few things have some addiction issues, Sulbutiamine has been reported to have withdrawal as does Caffeine although the latter without biological addiction)

Also, the 'inhibition of estrogen signalling' issue is not a good one, even for us guys. It inhibits the beta subunit and not the alpha subunit; this is bad

1.1. Sources and Cigarettes

Nicotine is the primary tobacco alkaloid (with minor alkaloids being nornicotine, anatabine, and anabasine) and exists in tobacco leaves to act as a pesticide, to kill insects that attempt to feed on the tobacco plant (a similar origin as a 'phytoalexin' to both Resveratrol and Caffeine),[1] it comprises up to 1.5% of the total weight of tobacco sold commercially in cigarettes and 95% of the total alkaloid content.[1] An average cigarette contains 10-14mg Nicotine[2] yet only 1-1.5mg reaches circulation after smoking a standard cigarette.[3]

Most alkaloids found in tobacco are unique to tobacco and structurally related to nicotine, including myosmine, N'-methylmyosmine, cotinine, nicotyrine, nornicotyrine, nicotine N'-oxide, 2, 3'-bipyridyl, and metanicotine.[1] Being exclusive to Tobacco does not apply to the alkaloid Myosmine, which is fairly ubiquitous in the diet[4] and to a small degree nicotine is also found in plants of the solanaceae family, although only at 2-7mcg/kg vegetable.[5] Population exposure to nicotine via these vegetables averages to 1.4mcg per day, with the 95th percentile ingesting 2.25mcg daily from vegetables; this is approximately 444 times lower than the lowest estimate from a single cigarette.[3]

Nicotine is the primary alkaloid found in the tobacco plant, and also exists in plants of the Solanaceae family such as eggplants, potatoes, and tomatoes at a level too low to cause the neurological effects associated with nicotine smoking

2.1. Absorption (Smoking)

Nicotine is normally a weak base with a pKa of 8.0 and in acidic environments where nicotine tends to exist in its ionized state it cannot easily cross membranes. Acidic environments prevail with many flue-cured cigarettes (pH 5.5-6.0) and as such nicotine does not easily cross the buccal (mouth) membrane in these instances.[6] Some transport does occur as the tar droplets nicotine is carried on may have a more alkaline pH though,[7] but most absorption in these instances occurs in the respiratory tract.

Nicotine can cross the buccal later when the pH is more alkaline, and this occurs with air-cured tobacco common to pipes and cigars (rather than the aforementioned flue-cured common to North American cigarettes) the nicotine is commonly unionizable and can cross the buccal membrane.[8]

In regards to the lungs nicotine is absorbed when it comes into contact with the lung alveoli; absorption appears to be rapid possibly due to the large surface area of the alveoli paired with the pH of the lungs being 7.4 and facilitating to nicotine membrane transport.[1]

2.2. Absorption (Other)

Chewing tobacco, gum and snuff are buffered to make their pH more alkaline, to facilitate transfer across the buccal layer.[9] This buffering also occurs with nicotine patches to facilitate transdermal delivery.[10]

The overall bioavailability of nicotine in gum form is lesser than that of inhaled, and measured at approximately 50-80% of the total content of the gum. This lesser bioavailability is due to some intestinal absorption of nicotine from swallowed saliva, which is subject to first-pass metabolism.[11]

Nicotine patches vary on their absorption depending on brand, although each patch commonly has a 1 hour lag time prior to nicotine being introduced to systemic circulation, with some residual (10% of the patch) still being released into systemic circulation after the patch is removed due to nicotine still being released from the skin.[12]

2.3. Serum Kinetics

In some studies on smoking cigarettes, the Tmax of nicotine (time to peak concentration) appears to coincide with finishing the cigarette, while oral snuff and chewing tobacco are slightly delayed (and not as easily titrated) and nicotine gum fails to reach the same Cmax (high concentration) as an equal dose of nicotine via chewing tobacco or cigarettes.[9] The first neural peak of smoked nicotine appears within 10-20 seconds after a cigarette puff,[13] but the exact amount of nicotine that is counted at these time points is variable due to puffs themselves being variable (longer puff, rates of puffing, dilution of inhaled puff with room air)[1] although the range of nicotine reaching systemic circulation has once been averaged to 1-1.5mg nicotine[3] when considering normal smoking habits using an average North American cigarette of 10-14mg nicotine.[2]

Chewing gum containing nicotine at 6mg is expected to increase circulating levels of nicotine in the range of 15-20ng/mL, whilst smoking a cigarette may elevate levels to the range of 15-30ng/mL.[14]

2.4. Distribution

In the blood, the pH being 7.4 denotes that nicotine is in a 69% ionized and 31% deionized form with plasma binding to proteins being less than 5%.[15] The steady state volume of distribution averages 2.6L/kg and is extensively distributed to the body.

The highest organ affinity for nicotine appears to be the liver, kidney, spleen and lung and the lowest adipose (fat mass), as assessed by autopsies from smokers.[1] It reaches skeletal muscle at the same concentrations as that found in the blood, and it can bind to brain tissue with high affinity and increasing receptor binding capacity in smokers relative to nonsmokers.[16][17]

Nicotine builds up in bodily fluids, particularly saliva and gastric juices due to ion trapping[18] and can also accumulate in breast milk at a 2.9:1 milk:plasma ratio.[19] Nicotine also crosses the placental barrier easily and can accumulate in amniotic fluid to a level slightly higher than serum,[20] and can gain access to a fetus if ingested by a mother.

2.5. Neural Kinetics

Due to the rapid transit of smoke to the lungs as well as the rapid absorption of nicotine in the lungs, nicotine can be detected in the brain tissue within 10-20 seconds after a puff of a cigarette which has been measured as being faster than intravenous injections.[13] This rapid neural transit of nicotine paired with both nicotine's ability to cause dependence based on context of the reward (See Neurology section) and the ability of a smoker to titrate cigarette effects to personal preference makes cigarettes the most addicting method of nicotine administration.[1][21]

Volume of Distribution of nicotine to plasma (100% being equal ratios) appears to be around 20% for the whole brain (nonsignificant from this primeape study[22]) with preferential partitioning into the preoptic area (29%) and amygdala (39%) with less distribution into white matter (10%),[22] but this study used an aromatase inhibitor as assessment while primape distribution of Aromatase rivals the distribution noted above (although in humans, a large thalamic content of aromatase exists).[23][24]

Smoking nicotine appears to be the most neurological reinforcing method of nicotine administration due to its kinetics and titrating the dose according to individual needs

2.6. Metabolism

Nicotine undergoes vast metabolism in the body which varies by pathway, although its main metabolic pathway is via continine (70-80%)[1][25] although 10-15% of all urinary excretion products are continine,[26][27] the majority of metabolism goes via continine but continues on into further metabolic pathways.

Direct conversion of nicotine to cotinine is mediated via the intermediate the nicotine-Δ1'(5')-iminium ion; the conversion from nicotine to this intermediate is mediated by the P450 enzyme CYP2A6 and conversion to cotinine is mediated by cytoplasmic aldehyde oxidase.[1] Continine can then be glucuronidated and excreted in the urine as Continine Glucuronide, converted to Continine N-oxide, or converted to trans 3-hydroxycontinine (which can then be glucuronidated and excreted in the urine).[1] It should also be noted that nicotine per se can also be glucuronidated and excreted in the urine as nicotine glucuronide, and this accounts for 3-5% of administered nicotine in humans.[26]

Beyond the 10-15% of metabolism accounted for by free continine and 3-5% by nicotine glucuronide the breakdown of metabolites appears to be around 33-40% trans–3'–hydroxycotinine (largest metabolite), 12-17% as continine glucuronide, and 7-9% as trans–3'–hydroxycotinine glucuronide.[1]

Major pathway of nicotine metabolism is via conversion into Continine, and then either unchanged continine is excreted in a detectable amount of further metabolism into continine metabolites are undergone; glucuronidation (having a glucose attached to the molecule) can occur at either nicotine or continine, as well as one of continine's metabolites

Another route that is responsible for 4-7% of metabolism is Nicotine N'-oxide, which is the result of nicotine being subject to a flavin-containing monooxygenase 3 (FMO3)[26] and mostly producing the trans isomer of Nicotine N'-oxide in humans.[28][29] This is a urinary end product and trans Nicotine N'-oxide can be found in the urine[29] or reduced back into nicotine in the intestines, and this metabolite paired with basic Nicotine glucuronidation (3-5% overall nicotine ingestion) account for most of what is left after the cotinine pathway is accounted for.[1]

Finally, two other non-oxidative pathways of nicotine metabolism are conversion to the N -methylnicotinium ion when the pyridine nitrogen is methylated[1] which can be detected in the urine as a byproduct,[30] and conversion of nicotine to nornicotine,[31] nornicotine of which is also a component of tobacco.

2.7. Enzymatic Interactions

Aromatase (CYP1A1/2) appears to be inhibited by nicotine, with an IC50 value of 223+/-10uM[32] and while it is twice as potent as the nicotine metabolite cotinine, they are additive in inhibiting aromatase.[33] Inhibition of Aromatase by nicotine and cotinine is reversed with supraphysiological levels of androstenedione.[34]

Other inhibitors or Aromatase in tobacco include Myosamine (IC50 33+/-2uM; 7-fold more potent than Nicotine), [32] Anabasine,[34] N-n-octanoylnornicotine[35] (comparable potency to aminoglutethimide, a pharmaceutical[36]) and N-(4-hydroxyundecanoyl)anabasine.[37]

Nicotine inhibits aromatase. Fairly weakly when looking at the concentration required to inhibit 50% of enzymatic activity but other compounds in tobacco appear to be more potent

That being said, one study using intravenous nicotine administration to baboons (at levels to mimic cigarette smoking; 0.015-0.3mg/kg) inhibited aromatase in the brain[22] and appears to be biologically relevant to neurology.[38]

3.1. Neurophysiology

Nicotine ingestion (in smokers) act to increase neuronal activity in the frontal and cingulate regions of the brain as well as the nuclear accumbens and amygdala; areas of the brain involved in addiction patterns.[39]

3.2. Attention and Reaction Time

A meta-analysis on nicotine and its cognitive effects in humans concluded that there was significant evidence for nicotine in enhancing attention (both alerting and orienting subsets, the former of which is 'maintaining an alert state' and the latter 'directing attention to sensory events').[40] This meta-analysis is more suited to answer questions on nicotine per se as previous meta-analysis'[41][42][43] used smokers as subjects and could only conclude the cognitive enhancing aspects of nicotine in a state of nicotine withdrawal (as a washout period preceded many trials). Said meta-analysis[40] looking at only laboratory studies in healthy persons and excluding any of nicotine-deprived smokers or those not double-blinded against placebo was able to find 41 studies in total and was able to analyze the parameters of alerting attention (accuracy and reaction time) as well as orientating attention (accuracy and reaction time), 76% of included trials noted no affiliations to the tobacco industry nor did the meta-analysis.

Of these, nine studies assessed alerting attention accuracy[44][45][46][47][48][49][50][51][52] and 8 of these studies plus 5 others assessed reaction time for alerting attention.[53][54][55][56][57] Only 5 (unique) studies assessed the accuracy of orientating attention[58][59][60][61][62] while these 5 and an additional 6 (two studies under one citation[63]) assessed the reaction time to orientating attention.[64][65][66][67] For these, significant and positive effect sizes were seen on alerting accuracy (g=0.34, z=4.19, p less than 0.001) alerting reaction time (g=0.34, z=3.85, p less than 0.001) and orientating reaction time (g=0.30, z=3.93, p less than 0.001) with a non-significant benefit to orientating accuracy (g=0.13, z=0.47, p less than 0.6).[40] A rough linear dose-response curve was noted for these parameters.

Reliable improvements in attention are seen with various doses of nicotine in a somewhat dose-dependent manner. Improvements in directing and keeping attention on stimuli are seen with improved accuracy, and improvements are seen in switching attention between stimuli but the accuracy on switching may not be as reliable

3.3. Anxiety and Depression

In a study on minor cognitive decline in nonsmokers, it was demonstrated that 15mg nicotine patches daily for 6 months was associated with improved scores on self-rated Worries and Anxiety subscales, indicative of an anxiolytic effect.[68] This same study noted no significant influence on the Depression subscale.[68]

One study using nicotine in non-smokers noted that 2mg Nicotine (gum) noted that the brain regions associated with adverse perceptions had enhanced activity under the influence of nicotine relative to placebo, suggesting that nicotine may enhance anxiety-inducing perceptions.[69] Coupling of the amygdala and perigenual ACC increased in the nicotine group under unpleasant conditions while decreasing under pleasant conditions, and led to increased state anxiety (subjective rating).[69] A possible confound in this study is participants being blinded, but accurately guessing whether they were in the nicotine group or placebo; possibly influencing results.

3.4. Aphrodisia

One study testing basic cigarettes against nicotine-free cigarettes noted that sexual effects manifested from blood flow (the measurement in this study was penile diameter) were negatively influenced by the nicotine containing cigarette only, suggesting nicotine could be an anti-sexuality compound.[70] Two studies later conducted in non-smoking men[71] and women[72] both noted that nicotine was able to reduce sexual stimulation (assessed via pornographic films and self-reported surveys) without significantly influencing any other mood parameter; men also reported less erectile potential after nicotine.[71]

3.5. Cognition

Improvements in memory, specifically short-term episodic memory, have been found to be significant in a meta-analysis on nicotine.[40]

One 6-month study on persons with Mild Cognitive Impairment (persons over the age of 55 with self-reported memory lapses) noted that nicotine patches at 15mg daily (released over 16 hours) were associated with improvements in memory, attention, and psychomotor speed despite no differences in global ratings of improvement by clinicians.[68]

3.6. Fatigue

Nicotine has been shown to reduce neural fatigue when the subjects are stratified to a group with higher baseline impulsivity (lower self control), with no significant effect in persons with lower impulsivity.[73]

3.7. Reward

In a study of non-smokers, nicotine patches at 14mg (two 7mg patches) were able to enhance the reward response to non-drug stimuli.[44] This study used a difficult computer visual test (requiring differentiation of 1.5millimeters under 100milliseconds) and rigged the response to give a 3:1 win:loss ratio with some of the rewards coming with minor monetary compensation and a greater response to reward was seen with nicotine and appeared to be long-lasting (still influencing decisions during the second part of cross-over 1-2 weeks later).[44] This general trend for nicotine increasing rewards for non-drugs have also been replicated in current smokers rewarded money after a trial.[74]

These results are paralleled in animals, where introduction of nicotine is associated with increased reward response to non-drug stimuli[75] and withdrawal from nicotine is associated with less reward from non-drug stimuli.[76]

3.8. Impulsivity

In a study on smokers who were also gamblers, it was noted that despite 4mg nicotine (via inhaler) being able to suppress cravings for cigarettes that it did not have any influence on gambling behaviour relative to placebo.[77]

One other study investigating the nicotinic acetylcholine receptor (of which nicotine is an activator) using 7mg transdermal nicotine (patches) and assessing impulsivity via three tests (stop signal (SS) delay, SS reaction time, standard deviation of go reaction time) noted that nicotine improved impulsivity in the group with higher baseline impulsivity (lesser self-control) while having no significant effect in low impulse persons.[73] Measures of reaction time varied and benefitted the low impulse group.[73]

4.1. Mechanisms

The current theory for mechanisms behind nicotine dependence is activation of nicotinic acetylcholine receptors (nAChRs) on mesocorticolimbic dopaminergic neurons,[75][44][78] which thereby increases responsiveness to rewards and motivation,[79] and non-drug rewards as well.[75][44] Through these mechanisms, nicotine is seen as a reinforcer of learned (conditioned) behaviour.[78]

Secondary to activation of α4ß2 and ß2 nAChR receptors on dopaminergic neurons in the VTA, they are depolarized causing an increase in overall firing and burst firing rate.[80][81][82] Some direct activation on α4ß2nAChRs directly excite these dopaminergic neurons.[78] All these mechanisms result in an influx of dopamine into the nuclear accumbens, which is a process highly associated with addiction as other addictive drugs such as heroin or cocaine possess this reaction[83] and inhibiting this dopaminergic process reduces cravings associated with nicotine.[84][85]

Activation of the a7nAChR receptors acts to increase excitatory drive towards the nuclear accumbens from the Ventral tegmental area (VTA) as well as two other areas known as the pedunculopontine tegmentum (PPT) and laterodorsal tegmentum (LDT)[86] as binding to presynaptic a7nAChRs (commonly on glutaminergic afferants extending to dopaminergic neurons[87]) enhances glutaminergic actions on these afferants and facilitates long-term potentiation.[88][89] Unlike α4ß2 and ß2 receptors which are fairly quickly desensitized after activation[82] the a7nAChRs are slow to desensitize which is conducive to their long-term potentiation via enhancing glutaminergic signaling.[82][90]

In many practical scenarios, the inhibitory potential from GABAergic neurons is somewhat lessened. GABAergic neurons, which are expressed plentifully in the VTA[91] and normally antagonize glutaminergic excitation, express mostly α4ß2 receptors.[92][93][78] In situations where smokers routinely ingest nicotine and keep elevated levels, these receptors desensitize and their effects lessen due to less α4ß2 activation, which leads to a relative spike in a7nAChR reinforcement and activation on glutaminergic neurons.[78]

Activation of dopaminergic neurons directly is associated with many of the acute effects of nicotine in this brain area, while activation of the a7nAChR on neurons extending towards this brain region eventually facilitate a reinforced neural network and is a mechanism of long term addiction. Mechanisms are in line with the notion that prolonged exposure to consistently high levels of nicotine is associated with greater addiction risk

Enhanced dopamine release has been noted in dependent smokers, and was seemingly absent in non-smokers in this study.[94]

When comparing nicotine per se against tobacco cigarettes in dependent smokers, preload with a 4mg nicotine lozenge against placebo and then smoking the nicotine-free cigarette in both groups determined that the smoking of the cigarette regardless of nicotine was associated with pleasure and a reduction of cravings and that nicotine pre-load reduced the amount of self-administered puffs given and further reduced the craving.[95] The ability of nicotine-containing tobacco but not nicotine-free tobacco has been noted elsewhere.[96]

4.2. Kinetics

One aspect of the rewarding portion of nicotine is the speed of which it reaches the brain, and association with perceived rewards. Smoked nicotine can reach neural tissue in 10-20 seconds,[13] faster than intravenous injections and this is matched by intra-nasal administration of nicotine.[97][98] The rapid increase in neural concentrations of nicotine are a contributing factor to the addictive properties of cigarettes with most reinforcement value.[21] Other forms of nicotine administration that avoid such a rapid and drastic Cmax in neural tissue (gum, patches, sublingual tablets, and lozenges) are associated with less dependence, although the lower rates of dependence from these products also correlates to the amount of nicotine absorbed.[10]

Dissociating nicotine spikes in the brain with cigarettes may per se reduce addiction, as pairing a 21mg nicotine patch (slow-release) with cigarettes that are free of nicotine may ease transition into a cigarette cessation program.[99]

The speed at which nicotine reaches the brain and the overall concentration of nicotine that reaches the brain are predictors of the addicting potential of nicotine, with high doses and fast absorption (cigarettes) being more addictive than slower release forms (gum, patches)

One intervention carried out in smokers wishing to quit noted that in groups assigned to nicotine gum (2mg or 4mg; n=127) a 15mg transdermal patch (15mg; n=124) nasal spray (n=126) or a nicorette inhaler (n=127) with ad libitum intake of the products noted that, in users that were abstinent for at least 3 weeks and completed the trial at 12 weeks, that all interventions were equally effective in the rates of smokers who remained abstinent and the average pleasantness or satisfaction during this period.[10] Rates of dependence on the nicotine replacement therapies were judged by how many persons were still using 3 weeks after the trial ended (37% of the spray group, 28% of the gum group, 19% of the inhaler group, and 8% of the patch group) and self-reported ratings of dependence at this time-point (33% inhaler, 22% gum, 20% nasal spray, 0% patch); when looking at all time points, the gum had less self-reported dependence than both inhaler and nasal spray while the patch had the least reported dependence.[10]

Nicotine replacement therapies are still associated with dependence, which is related to speed and overall amount of nicotine absorbed. These rates of dependence are less than that seen with cigarettes

4.3. Sex

There appears to be a sexual dimorphism when it comes to addiction to nicotine (in this section, sometimes used interchangeably with cigarettes), as female smokers appear to require less overall exposure to nicotine/smoking to become dependent and have more difficulty quitting.[100] These differences appear to have biological basis, as studies in laboratory animals note these sexual differences repeatedly, where low-dose nicotine (verging on a level where rats may not self-administer, indicative of addiction) appears to affect more females than males while females being reported to go to greater lengths to acquire nicotine after dependence has developed.[101][102][103][104][105]

It is thought that circulating hormones may play roles, as exogenous progesterone is associated with attenuating cravings and pleasure from smoking.[106] Additionally, interactions with the estrus cycle have been noted as it pertains to nicotine dependence where women report more smoking during menstruation[107][108] and this has been noted to be independent of menstrual symptoms (smoking for the purpose of relieving symptoms of menstruation, for example).[109] Some studies do fail to note these associations, however.[110][111] Sensitivity to withdrawal appears to be greater during menstruation or shortly after.[112][113][114]

Nicotine interacts significantly with hormones in the estrogen group as well as progesterone

These interactions may be downstream of Nicotine being able to perturb estrogen signaling in neural tissue, directly inhibiting the beta subunit of the estrogen receptor[115] and inhibiting aromatase respectively (more information on these topics in the Hormones subsection of Estrogen, and the Pharmacology's subsection of Enzymatic Interactions)

4.4. Dependency

Cigarettes (not nicotine per se) are used by 19.8% of the American population (2007 stats[116]) and despite 45% of current smokers having to tried to quit (2008[117]) only 4-7% were likely successful.[118] During smoking cessation, one of the most common side-effects reported is difficulty concentrating[116] and one of the more commonly reported reasons that smoking is continued is for the perceived cognitive benefits attributed to nicotine.[119] Due to these reasons, nicotine has long been investigated for its role in the well-established dependency on tobacco-containing cigarettes.


Edit5. Cardiovascular Health

5.1. Heart Rate

6mg of Nicotine gum, given to 21 year old men, is able to increase heart rate as well as both diastolic and systolic blood pressure 30 minutes after consumption,[71] this study design replicated in women also noted an increase in heart rate but failed to establish a significant increase in blood pressure.[72]

A 6-month study using 15mg nicotine patches noted a significant decrease in blood pressure, where placebo reported an average 9.6mmHg increase in systolic over 6 months whereas nicotine patches recorded a reduction of 4mmHg systolic.[68]


Edit6. Interactions with Glucose Metabolism

6.1. Inflammation and Glucose Metabolism

Secondary to Nicotine's anti-inflammatory effects, there is potential for nicotine to induce insulin sensitivity if the underlying mechanism of resistance is from excessive inflammation, and this occurred in rats without alterations in body weight.[120]

6.2. Interventions

Cigarette smoking per se can negatively influence glucose metabolism,[121][122] and long-term usage of nicotine gum is correlated with insulin resistance.[123] Due to this, the effects of nicotine per se are of interest for researchers.

When looking at the effects of nicotine in isolation in healthy smokers noted that a 14mg transdermal patch of nicotine resulted in nonsignificant trends for increased insulin resistance and higher blood glucose levels.[124] This test used a blinded patch procedure and administered an oral glucose tolerance test 3 hours post-patch and measured parameters for 2 hours.[124]

In non-smoking individuals administered a nicotine infusion, it appeared that the baseline glucose uptake rate in healthy individuals (10.9+/-0.3mg/kg LBM) was wholly unaffected by nicotine while that of type II diabetic persons was aggravated by approximately 32+/-6%, indicating that nicotine differentially affects healthy and diabetic persons.[125] This supports previous research that suggests that nicotine administration to diabetics aggravates insulin resistance[126] while a study using snuff noted that in healthy persons snuff per se failed to induce insulin resistance while smoking did; implicating a compound in cigarettes but not snuff and thus not implicating nicotine per se.[121] In this study that differentiated between 'healthy' and 'diabetic' smokers, it did so based on circulating glucose, insulin, and HbA1c (all higher in diabetics) although insulin was titrated to control for blood glucose levels; nicotine administration was 0.3mcg/kg/min to simulate a cigarette.[125]

Nicotine appears to aggravate insulin resistance in the already resistant, which works negatively in conjunction with cigarettes (that appear to induce insulin resistance), there is some evidence that this adverse effect is either lost or just does not apply to persons with good insulin sensitivity and may only apply to diabetics

6.3. Insulin Sensitivity after Smoking Cessation

A known phenomenon with smoking cessation is a gain of weight, usually body fat; this tends to be a mixture of reduced metabolic rate paired with excessive caloric intake although it is attributed in part to increased insulin sensitivity after smoking cessation.[127] Nicotine patches do not appear to interfere with the regain of insulin sensitivity that occurs after smoking cessation.[128]


Edit7. Obesity and Fat Mass

Cigarettes are known to stimulate lipolysis (fat burning) after being smoked[129][130] and can be mimicked by intravenous administration of a similar level of nicotine;[131] when comparing monozygotic twins, smoking twins tend to be 2.5-5.0kg lighter than non-smoking counterparts.[132] Although many possible reasons exist for these observations, the stimulation of lipolysis and cholinergic neuron innervation of adipose tissue[133] implicates a direct fat burning effect of nicotinic acetylcholine receptors.

7.1. Mechanisms

Nicotine is able to induce AMPK activity in adipocytes, which is associated with a time and concentration dependent increase in lipolysis;[134] as the increase in AMPK and lipolysis was inhibited by N-Acetylcysteine, it was said to be mediated by pro-oxidative effects.[134] Oxidant stress is known to regulate AMPK, particularly peroxynitrate (pro-oxidative derivative of Nitric Oxide)[135] and these effects occur at levels of circulating nicotine that can be reached after a cigarette (as low as 6nM, increasing up to 600nM).[134] However, this activation of AMPK does not cause lipolysis by nicotine (as the inhibitor, Compound C, successfully inhibited AMPK but failed to abolish lipolysis).[134]

The increase in lipolysis observed by nicotine appears to be related to nicotine suppressing Fatty Acid Synthase (by 30% at 100nM), which may have been secondary to peroxynitrate,[134] and a possible increase of catecholamines such as adrenaline which are released in response to nicotine (shown in humans after intravenous administration).[136][131] This study noted that 7.2ng/mL nicotine (achievable levels after a cigarette) increased adrenaline and noradrenaline by 213+/-30% and 118+/-5% respectively, and the release of glycerol (144-148%) was inhibited by a cholinergic agonist (thus acted through acetylcholine receptors) and attenuated by 60% by propanolol (a beta-adrenergic antagonist, implicating the catecholamine release). A reduction of nicotine-induced lipolysis has been noted elsewhere with blocking the beta-adrenergic receptors.[129]

Nicotine acts on acetylcholine receptors to release adrenaline and noradrenaline, which then act on beta-adrenergic receptors (molecular target of adrenaline and Ephedrine) to induce fat loss. This is not the only mechanism, but appears to be the major one

Activation of the nicotinic acetylcholine receptor on fat cells is associated with reduced secretion of pro-inflammatory TNF-a,[137] and this receptor (specifically a7nAChR) appears to be negatively correlated with body fat mass; with those having a BMI of 40 or above having up to 75% less mRNA and protein content than normal weight individuals.[138]

Activation of the nicotinic acetylcholine receptor in fat cells mediates anti-inflammatory effects in the fat cell, and less secretion of pro-inflammatory cytokines

7.2. Metabolic Rate

In otherwise healthy men, chewing nicotine gum containing 1-2mg of nicotine increases metabolic rate (indirect calorietry) via 3.7-4.9%, and this appears to be increased with coingestion of 50-100mg caffeine in the gum with no dose-dependence seen with the caffeine addition.[139] Fat oxidation rates with nicotine were not changed relative to control, and measurements were taken over 180 minutes where the first 25 involved gum chewing.[139]

7.3. Interventions

In rodents, nicotine is able to reduce body fat in both high-fat fed[140] and regular chow mice,[141][142] both of which are blocked by the acetylcholine receptor antagonist mecamylamine; one study noted that selective inhibition of the α4ß2 receptor (with varenicline) only partially inhibited the fat loss.[140]

It has been demonstrated in rats that a reduction in body fat exists when food is controlled for, excluding a reduction in food intake.[142][141] However, these studies tend to use very high doses of nicotine (2-4mg/kg, one study using up to 4.5mg/kg which is equivalent to 2.5 packs of cigarettes). The changes have been noted with doses as low as 0.5mg/kg oral ingestion and are dose-dependent, but may not be statistically significant over time (as efficacy appears to be attenuated).[140] One study in male smokers (who are desensitized to the effects of nicotine) given 4mg of nicotine gum or the equivalent dose in cigarette or inhaler failed to notice an increase in lipolysis over 180 minutes of measurement, and this study did not note any increases in adrenaline.[143]

In regards to metabolic rate, several studies have noted an increase in rodents with isolated nicotine [141][144][145] whereas in humans cigarettes have been tested, and found to increase metabolic rate by 210kcal over 24 hours relative to non-smoking controls.[146] This increase in metabolic rate may simply be mediated via the increase in adrenaline and noradrenaline as it has a half-life of 3.5 minutes (similar to the active half-life of the adrenaline receptors),[147] despite the increase in lipolysis not having an apparent half-life.

Animal interventions note a reliable increase in lipolysis and metabolic rate, which is attenuated over time (low dose nicotine eventually becoming not significantly different than placebo, with higher doses required to maintain lipolysis). Increase in metabolic rate may simply be due to more catecholamines (adrenaline and noradrenaline)

One study using nicotine patches in 55 year old persons noted that weight was reduced by 1.3kg 91 days after administration (placebo down 0.13kg) yet when measured again at 6 months this apparent difference faded and was no longer significant.[68]

Human interventions assessing nicotine in isolation over time for the purposes of weight loss are not too prevalent

7.4. Rebound Weight Gain

A known phenomenon with smoking cessation is a gain of weight, mostly body fat, which tends to be due to a reduced metabolic rate paired with increased food intake.[127] Nicotine itself is (weakly) implicated in attenuated the weight regain from cigarette cessation, but with relatively mixed results despite most survey research suggesting it is beneficial to attenuate weight gain.[148][149][150][151]

A failure of attenuating weight regain has been noted despite increased smoking cessation rates with nicotine gum (2mg gum; no limitation of dose).[152][153][154] One study has noted benefit with 2-4mg when an hourly dosing protocol was issued,[155] suggesting a possible dose-dependent effect (which was later not observed with patches, when this study was structured to assess dose dependence).[156]

When looking at compounds that do appear to have efficacy in reducing weight rebound during smoking cessation one Meta-Analysis noted that naltrexone, dexfenfluramine, and phenylpropanolamine had some evidence[127] as well as fluoxetine.[157]

Nicotine does not appear to inherently exert much promise for attenuating weight regain during smoking cessation. The inherent weight regain that is associated with smoking cessation (of which nicotine replacement therapy encourages) could potentially explain part of this, as could desensitization to the appetite suppression from nicotine


Edit8. Skeletal Muscle

8.1. Mechanisms

Nicotine has once been demonstrated to activate mTOR when incubated in skeletal muscle cultures,[158] which was thought to underlie the reduced insulin sensitivity associated with smoking (as mTOR activation induces IRS-1 and suppresses insulin signalling).[159]


Edit9. Interactions with Inflammation

9.1. Mechanisms

Nicotine appears to exert anti-inflammatory properties via its actions as a cholinergic agonist.[160] Upon activating the a7 nicotinic acetylcholine receptor (a7nAChR) on immune cells, particularly dendritic cells[161] and macrophages.[162] This pathway is naturally regulated by the neurotransmitter acetylcholine, which upon release from the vagal efferents in the vagal nerve[163] it inhibited the ability of immune cells to respond to TNF-a[164] and attenuated its release from immune cells.[164] This was later found to also inhibit NF-kB activation in LPS-activated macrophages, the standard anti-inflammatory in vitro test[165] and can also exert this effect in splenocytes.[166]

Overtly, activation of nicotine's receptor by either nicotine itself or the neurotransmitter acetylcholine can suppress inflammatory reactions in immune cells and reduce the secretion of pro-inflammatory cytokines

Precisely, Nicotinic activation of a7nAChR increases expression of JAK2 and STAT3,[167] which in turn induce the expression of tristetraprolin (TTP) which acts to destabilize TNF-a and prevent its actions.[168][160] TTP is a fairly potent cytoplasmic regulatory of inflammation, and its knockout induces arthritis in rats.[169]

Another possible mechanism is suppression of High Mobility Group Box 1 (HMGB1) which is a possible mechanism for how nicotine can aid in reducing clinical signs of sepsis and, in animals, even after sepsis was induced (implicating rehabilitative properties rather than preventative).[170][171]

9.2. Ulcerative Colitis

It was observed in epidemiological research that smokers have less risk for ulcerative colitis, with a relative risk of 0.6 (0.4-1.0) when paired with nonsmokers and past smokers having double the risk relative to current smokers (1.1-3.7).[172] This observation has been noted elsewhere in case control studies,[173][174] and does not extend to other intestinal diseases such as Crohn's[175] (sometimes being associated with increased risk[176]) and may not extend to IBD (Inflammatory Bowel Disease).[177] It has also been noted that persons who acquire Ulcerative Colitis tend to be past smokers rather than current smokers.[178][179][180]

These paradoxical effects are secondary to Nicotine acting as an anti-inflammatory alkaloid.[160]

Even when nicotine is consumed via cigarettes, there appears to be an inverse correlation where cigarette smoking is protective against Ulcerative Colitis


Edit10. Interactions with Cancer

10.1. Metabolites

N′-nitrosonornicotine (NNN), a nitrosamine found in tobacco as well as a metabolite of nornicotine,[14] appears to have carcinogenic potential.[181] NNN has been found in the urine of persons who quit smoking and were using either patches or gums containing nicotine, suggesting that some persons may produce NNN endogenously from nicotine.[182] An intervention addressing this issue using 21mg patches of nicotine over 24 weeks of smoking cessation noted urinary NNN noted that urinary levels ranged from below the detection limit (0.005pmol/mL) to 0.021pmol/mL with an average of 0.009+/-0.006pmol/mL.[183] This study also noted that 40% of nonsmokers exposed to second hand smoke (sample of 10) had a reported urinary level of 0.002pmol/mL NNN,[183] and although those two studies (the latter being well structured to assess this question) noted significant increases in urinary NNN, at least one study has failed to find an increase with nicotine replacement therapy (patches).[184]

Nicotine consumption appears to produce some degree of the carcinogenic N'-Nitrosonornicotine (NNN; via nicotine being metabolized to nornicotine, then to NNN), but the degree of urinary NNN production appears to be relatively minor. Urinary NNN is found in nonsmokers exposed to secondhand smoke as well, which studies have not controlled for (could potentially explain variability)

10.2. Lung

Activation of the α7 acetylcholine receptor appears to induce anabolic effects such as phosphorylation of Akt[185] and activation of Src,[186] with activation of the nicotine receptor apparently enhancing cytoplasmic markers of pro-inflammation (5-LOX, COX-2, and NF-kB translocation).[187][188]

Concentrations of 100nM nicotine, while not able to induce proliferation, may have anti-apoptotic effects.[189] The cholinergic receptors appear to act as a pro-survival signalling pathway in lung cancers,[190] which extends to acetylcholine itself.[191]

Activation of the nicotinic acetylcholine receptor appears to be pro-survival in cells, which extends to cancer cells; nicotine has the potential to interfere with apoptosis (regulated cell death) of cancer cells


Edit11. Interactions with Hormones

11.1. Testosterone

Both nicotine and its metabolite cotinine appear to appear to negatively influence testicular structure and circulating testosterone levels,[192] and may adversely influence the amount of androgen receptors expressed (rat study, prostate measured).[193] In part these mechanisms are secondary to testicular oxidation (inducing damage and reducing enzyme content) but some suppressing actions may be secondary to cholinergic agonism on the testes,[194] and similar mechanisms appear to exist for both nicotine and cotinine.[195]

One study administering 0.5mg/kg and 1mg/kg via gavage (into the stomach) for 30 days noted decreases in testicular weight associated with nicotine and no significant effect on prostate hypertrophy, decreases in circulating testosterone were noted in a dose-dependent manner but normalized after 30 days of nicotine cessation.[196] A study using a lower dose of 0.6mg/100g for 12 weeks also noted this decrease in testicular weight and suppressed both circulating and testicular concentrations of testosterone, both of which were half-way reversed the amino acid Taurine at 50mg/kg bodyweight and more-so with hCG.[197] Nicotine could decrease expression of 17ß-HSD and 3ß-HSD and reduce StAR expression to 60% of control, and this was mostly reversed with taurine and completely normalized with hCG.[197] Finally, another study using 20-week old mice (middle aged) administering nicotine at low doses of 0.0625mg/kg bodyweight (after a short introductory phase) noted that, over the course of 90 days, marked suppression of testosterone from 898.4ng/mL in control to 364ng/mL (59.5% decrease) in the nicotine group associated with (adverse) abnormal cell organization in the prostate.[198] Similar results have been noted before and the decrease in androgens was hypothesized to be the cause[199] although exact pathology is unclear.[198]

In rat studies where nicotine is administered, suppression of testosterone is seen at physiologically relevant doses which seems to be in part secondary to receptor activation (muscarinic cholinergic) and, in chronic situations, related to testicular damage secondary to pro-oxidation; damage is somewhat attenuated with anti-oxidants

In regards to human studies on the subject matter, one study in men who were deemed to be nicotine dependent smoking 15.48mg nicotine (to get 20ng/mL or above serum levels) failed to notice any changes in circulating testosterone when measured for 2 hours, although a trend to decreasing existed.[200] The only other study in Medline was a cohort survey of men aged 35-59 (n=221) who smoked daily prior to intervention, and their circulating levels of testosterone one year after abstinence; it was found that the measurements of testosterone at baseline and after a year of cessation were similar.[201] Larger scale surveys in older men (n=375, age 59.9+/-9.2) suggest that smoking is associated with increased testosterone, where after stratification into thirds based on serum testosterone and free testosterone level a higher percentage of smokers were in the top third relative to non-smokers or past smokers.[202] This correlation appears to hold true for adolescents as well as females, where testosterone appears to be higher in smokers.[203][204][205][206]

Other studies on a more intervention nature that measure testosterone may note no significant difference between groups, or even a trend towards higher testosterone levels in smokers (4.33+/-0.53ng/mL in nonsmokers, 4.84+/-0.37ng/mL in smokers).[207]

Lack of intervention studies conducted in humans, but all studies conducted assessing correlations (unable to establish causation) note higher circulating testosterone and free testosterone levels in smokers, this holds true across gender and age. This may be sufficient to suspect that testosterone does not reduce testosterone in practical settings, but cannot establish if nicotine increases testosterone

11.2. Estrogen

Nicotine has been shown to be an Aromatase inhibitor in vivo after baboons are injected with a concentration of nicotine (0.015-0.03mg/kg; reaching plasma levels of 15.6-65ng/mL) able to be reached after a cigarette,[22] which builds off previous research showing nicotine is active as an Aromatase Inhibitor in vitro[34][37] and may explain why heavy smoking women are susceptible to various estrogen-insufficiency related conditions (osteoporosis, menstrual disturbances, early menopause)[208][209] and to explain the higher than average circulating testosterone seen in smokers of both genders (which has currently not been demonstrated in acute interventions).[203][204][205][206][202]

The ability of Nicotine (and related nicotine alkaloids, when looking at smoking studies) to inhibit the Aromatase enzyme may cause a shift towards androgens rather than estrogens over time. The degree of change noted in these studies may be higher than can be accredited to nicotine due to other tobacco alkaloids

In studies that look at serum estrogen levels, one rat study noted that circulating estradiol levels were reduced when averaged out over 4 estrus cycles when compared to normal control as quickly as 4 days afterwards; showing some variability in degree of decrease.[210]

May reduce circulating estrogen levels in serum acutely, but interventions looking at this are surprisingly scarce

Damage via Ischemia (lack of oxygen) and Reperfusion (reintroduction of oxygen) is protected in part by estrogen signaling,[211] and this protection is removed under the influence of chronic nicotine administration.[212] A later study elucidating mechanisms noted that rats given 4.5mg/kg nicotine hydrogen tartrate (to mimic levels achieved by heavy smokers[213]) for 16 days prior to cerebral ischemia noted that nicotine could augment the damage done by cerebral ischemia (and while oral contraceptives were inert alone, they were synergistic in augmenting damage done by nicotine more than nicotine alone).[210]

These effects were thought to be through estrogen inhibiting intracellular estrogen signaling, and since the effect was mimicked by 1uM ICI 182780 it was claimed that nicotine inhibited the nuclear estrogen receptors and subsequent phosphorylation of CREB[210] which mediate the neuroprotective effects of estrogen (which inhibits NADPH oxidase and attenuates pro-oxidation in the cell);[214][215] nicotine appears to downregulate ER-ß rather than ER-a protein levels,[216][212] and this inhibition of ER-ß has also been implicated in reducing neuronal plasticity[217] and mitochondrial loss in neurons.[216]

Nicotine appears to act as an estrogen receptor antagonist, and can inhibit the signaling pathways of estrogen; It shows preference for ER-ß rather than the alpha subunit (the latter being associated with classical 'effects of estrogen')

The estrogen receptor subunit inhibited is the one associated with positive effects in both genders

11.3. Luteinizing Hormone

Luteinizing Hormone and Follicle-Stimulating hormone appear to be reduced by 40% and 28%, respectively, when rats are given 0.6mg/100g bodyweight nicotine for 12 weeks.[197]

One human study measuring LH for 2 hours after 15.48mg nicotine (smoked in dependent smokers) noted that LH spiked within 14 minutes of smoking a cigarette and was highly correlated (r=0.642) with serum nicotine levels.[200]

11.4. Prolactin

Cigarette smoking in dependent smokers is associated with an increase in prolactin levels within 6 minutes of the cigarette, remaining elevated for 42 minutes before returning to baseline at 120 minutes.[200]


Edit12. Nutrient-Nutrient Interactions

12.1. Caffeine

Usage of Caffeine and nicotine, usually via Coffee and tobacco, are positively correlated in society; smokers tending to also be persons who consume caffeine at higher levels or frequencies than nonsmokers.[218][219][220]

Caffeine and nicotine are widely used in together via common vessels of coffee/tea/energy drinks and cigarettes

Nicotine and caffeine, when coingested, appear to have additive thermogenic effects in heavy coffee drinking (440mg caffeine average) smokers (18.6-19.6 cigarettes daily).[221] When consumed alongside exercise, the thermogenic effects of caffeine and nicotine (combination) appears to be further enhanced but was found in this study to only occur in men.[221]

One study has noted that the addition of caffeine (50-100mg) to nicotine chewing gum (1-2mg) was able to enhance the appetite suppressing effects of nicotine, with the highest combination of the two (100mg caffeine with 2mg nicotine) causing nausea in some participants.[222]

Caffeine and Nicotine appear to have benefits together towards fat loss (due to additively being thermogenic, and synergistically suppressing appetite)

One study has noted that Caffeine (250mg) ingestion in smokers who did not consume caffeine for 4 weeks concomitant with high dose nicotine infusions reduced the subjective ratings of stimulant when compared to nicotine paired with oral placebo.[223] In persons who do not smoke but consume caffeine, there does not appear to be a significant interaction for neural properties of nicotine and caffeine.[224]

One trial assessing (by self-report survey) the effects of caffeine on reinforcing properties of nicotine failed to find any evidence that caffeine enhances addiction potential of nicotine when both are at moderate doses,[225] but these results are contrasted by a study using a monetary set-up (how much money participants disclosed that they would pay for injections of either compound) that found that caffeine's ability to reduce 'bad drug effects' of nicotine was evidence of reinforcing properties.[226] Nicotine replacement therapy (to attenuate cravings for nicotine) does not appear to have any crossover to caffeine withdrawal and cravings.[227]

Persons do not appear to self-report that they are more addicted to nicotine under the influence of caffeine, but may be more likely to use nicotine when caffeine is coingested (due to reducing 'bad drug effects' of nicotine)

12.2. Alcohol

Drinking Alcohol, known as ethanol, is a social drug used in similar demographics as many nicotine users. Their usage in society is highly correlated[228][229] and intake of nicotine appears to encourage intake of alcohol in social settings, usually in males.[230][231]

One study assessing nicotine and ethanol coingestion noted that nicotine (10mcg/kg) was able to significantly suppress self-reported intoxication from ethanol (breathe alcohol level of 40-80mg%) but exacerbated alcohol-induced memory deficits.[232] Sedation from alcohol can be abolished by nicotine[233] and enhance alcohol-induced euphoria,[234] which may lead to the less self-reported intoxication. This decrement of short-term memory processing has been reported before, and the group consuming the combination performs worse than placebo and ethanol alone,[235] and the memory-impairing effects of sub-effective doses of both when consumed suggest adverse synergism as it pertains to memory (this study performed in rats).[236]

Ratings on attention according to one study were not significantly influenced by alcohol, nicotine, or their combination.[232]

Appears to augment perceptual effects of alcohol such as euphoria and reduce sedation, yet alcohol appears to convert nicotine's cognitive enhancing effects into a short-term memory insult

12.3. N-AcetylCysteine

N-AcetylCysteine (NAC) is a bioavailable form of the amino acid Cysteine (found in high amounts in Whey Protein) that has been investigated for its role in reducing nicotine dependence.

The theory behind NAC and dependence is based on glutamate transmission, where relapse to addictive drugs is associated with less basal concentrations of extracellular glutamate. This leads to less activation of presynaptic mGluR2/3 receptors which normally suppress glutamate signaling, and lead to increased glutamate signaling secondary to less suppression (from less glutamate); although most mechanistic studies are done in models of cocaine[237][238][239] these receptors are activated during nicotine dependence and stimulating these receptors reduces the rewarding aspect of nicotine[240] and increasing extracellular glutamate levels attenuates withdrawal symptoms.[241] NAC is able to do the latter, increase extracellular glutamate, and slightly attenuate addiction to cocaine and heroin in rats.[242][243]

One double-blind trial investigating heavy smokers (15 or more daily) that abruptly ceased smoking, and then consumed either placebo of 3,600mg NAC in two divided doses failed to note any reduction of cravings with NAC and the reduction of side-effects trended towards but failed to reach significance.[244] However, when subjects were invited back to the lab to smoke the first cigarette to signal the end of the study, persons using NAC reported significantly less pleasure from this cigarette relative to control; on a scale of 1-100 placebo rated the cigarette as a 65.58+/-24.7 while NAC rated it 42.6+/-29.02 (35.1% less pleasurable).[244] This 'lesser reward' aspect may be of more use to current smokers, as one study (double-blinded) noted that NAC administration at 2,400mg daily for 4 weeks during a smoking lifestyle failed to note a significant reduction in cigarettes smoked per week per se but when social events were controlled (by factoring out cigarettes smoked on days with high alcohol intake) a significant reduction of cigarette smoking persisted; effects seemed to become more potent over the 4 weeks of use.[245]

May have a role in helping nicotine dependence by not necessarily aiding the dependency, but with reducing the pleasure attained from partaking in cigarettes. This reduction of pleasure is somewhat significant, but social context (drinking with buddies) may be enough to effectively override it

12.4. St.John's Wort

St.John's Wort (SJW) is a dopamine acting anti-depressant which has been investigated for its role in nicotine dependence due to showing benefits in mice[246] and, mechanistically, plausibly helping dependency via modulating catecholamines (dopamine, noradrenaline, adrenaline) and since buproprion (anti-depressant) appears to be effective as a smoking cessation aid.

The first open-label (not blinded) trials on SJW in nicotine dependence noted that SJW at 900mg daily for 3 months was associated with a 24% smoking abstinence rate at the end of the study.[247][248] This was followed up with a double-blind trial using SJW at 300mg and 600mg thrice a day (900mg or 1800mg total; 0.3% hypericin) for 12 weeks against placebo, where SJW failed to exert any significant differences against placebo.[249]

Results for SJW and treating nicotine dependence are looking lackluster and not too promising

12.5. Modafinil

Modafinil is an anti-sleep prescription medication for narcoleptics with cognitive-enhancing effects, and has been investigated for its role in reducing nicotine dependence. In one blinded trial, modafinil usage not only failed to exert increased abstinence rates for smoking cessation but increased the negative symptoms of smoking withdrawal.[250] Over 8 weeks and with 200mg Modafinil in the morning, quit rates were 44.2% in placebo and 32% in modafinil (nonsignificant difference) and modafinil reported significantly higher depressive symptoms and bad mood during nicotine withdrawal, with no significant difference in positive mood or desire to smoke.[250]

12.6. Taurine

Taurine is a non-essential amino acid that contains a sulfur group, it appears to attenuate (but not fully abolish) reductions in testosterone and other hormones (luteinizing hormone, FSH) seen with nicotine usage in rats.[197] It was investigated for this purpose due to being the most abundant free ß-amino acid in the male reproductive system[251] and having shown protective effects against nicotine on heart tissue[252] as well as bladder and urinary tracts,[253] thought to be secondary to its anti-oxidative capabilities.

May reduce the potential reductions in hormones due to nicotine, the 'reductions in hormones' not being the most reliable effect though and thus necessity of taurine not established

12.7. Ephedrine

One study giving nicotine (0.2mg/kg) to rats that failed to notice any adverse morphology on cardiac tissue with nicotine in isolation noted that the slight toxic signs associated with a combination of Caffeine and Ephedrine were exacerbated in the presence of nicotine;[254] this study used a fairly high dose of ephedrine (30mg/kg) but reasonable doses of caffeine (24mg/kg) and nicotine, as 0.2mg/kg in mice correlates to about 3mg in a 200lb human male.[255]

13.1. Interventions

An intervention using 15mg nicotine patches for a period of 6 months in persons who are otherwise healthy but over the age of 55 with slight memory impairment noted that overall reported of adverse effects were significantly higher in nicotine (82) than in placebo (52) yet none were rated as severe; blood pressure was reduced in this study alongside an improvement in cognition.[68]
